Mesotherapy is a medical technique invented in France by the general practitioner Michel Pistor. It consists of injecting low doses of medication under the dermis (the innermost layer of the skin).
The principle of Tunisian mesotherapy is to use smaller quantities of drugs by directly targeting the area to be treated or as close as possible.
Mesotherapy Mesotherapy: an effective and low toxicity therapeutic means
For a long time, diseases have been treated by acting directly on the affected area of the organism or sometimes on the organ itself.
Thus: migraines were treated by directly targeting the head, rhinitis by acting near the nose, arthrosis on the joints, sciatica along the leg and tendonitis as close as possible to the tendons... Everything happens as if the deep pathology is inscribed as a filigree on the surface of the skin.
It was not until 1958 that this method was called mesotherapy.
Mesotherapy Tunisia: the pathologies treated
Mesotherapy is initially intended to treat localized pain and venous insufficiency. It was mainly applied on points indicated by the patient as being closest to the pain.
This technique is nowadays used to treat many pathologies:
- Osteoarthritis ;
- Rheumatism;
- Lumbar or cervical pain;
- Tendinitis;
- Tinnitus;
- ENT infections;
- Hair loss (alopecia), etc.
- Sprains ;
- Shingles;
Nowadays the points are more precise and are not necessarily a projection of the deep pathology. Mesotherapy can be associated with acupuncture: in this case we speak of Meso acupuncture.
Mesotherapy Tunisia: conceptions of mode of action
The drug is deposited as close as possible to the target organ: this is the idea of PISTOR (see therapeutic challenge 3rd edition Malouine 1979).
Three hypotheses of mode of action:
- Mesotherapy would act thanks to the local or/and general stimulation of the microcirculation: schematically, a sick organ, tendon or joint would be in a state of microcirculatory suffering. Mesotherapy would correct this suffering.
- The Mesoderm Hypothesis: Mesotherapy is the treatment of connective tissue of mesodermal origin which includes microcirculatory, neurovegetative and immune systems. By acting on these three levels, mesotherapy can correct a functional disorder caused by the pathology in question.
- Energetic theory: In 1982 Daniel Ballesteros defined mesotherapy injection as an energetic message. This theory is based on acupuncture, auriculotherapy, energetic medicine and electromagnetism.
How to practice mesotherapy?
Most doctors practice systematic mesotherapy based on the theories of Dr. Robert MAIGNE and rheumatologist Didier Mrejen.
Dr. Robert Maigne relies on the determination of minor intervertebral disturbance and the search for painful areas on palpation (cellulo-tendino-myalgic syndrome) in relation to the disturbance of a spinal stage.
According to him, these zones are found at the level of the endings of the posterior, collateral and anterior branches of the disturbed stage.
Didier Mrejen proposes neurophysiological bases of dermatological zones and an original cutaneous cartography of both spinal and visceral suffering. For him, all somatic suffering (joint, muscle, water ligament) is represented on the skin by a palpable, painful, reproducible zone.
These dermo neuro dystrophic zones that should be searched for are those that should be chosen to apply a meso-punctual technique.
The mesotherapy procedure is based on 3 questions: Where to prick? With which products ? and with which technique (material, depth, quantity of mixture injected).
We prick in orthogonal projection of the presumed lesion and/or the pain described by the patient.
We also prick in the areas of dermo neurodystrophy described by Dr Didier Mrejen for all spinal pathology as well as for visceral pathology accessible to mesotherapy.
The choice depends on the lesion to be treated and the intensity of the pain.
Examples:
- Acute tendinitis: Non-steroidal anti-inflammatory drugs (NSAIDs)
- Chronic and rebellious tendinopathy: vasodilator
- Dermo neurodystrophy area of inflammatory appearance :NSAID
- Area of dermo neurodystrophy of cardboard-like appearance: vasodilator
- In case of severe pain: analgesic level 2 and NSAIDs.
- The 4 mm long and 4/10 diameter needle is perfectly adapted to the maximum of pathologies. For thick skin we can use the 8mm needles
- The electronic gun is less painful. It is mainly used for sensitive athletes
- Other means are cited but they are little used:
- multi mechanical injectors
- electronic multi-injectors
- Mesoperfusers (mesodermic infusion over 10 minutes)
- mesotons (iterative injection of 0.1 ml at each pulse)
- mesalysis: faster and more accurate single-use method
The most used products in mesotherapy.
Cortisone products are not used because they alter connective tissue and capillaries. The most frequently used products are:
- NSAIDs: voltarene, feldene, profenid, indocid, aspegic
- Myoreorelaxants: coltramyl is best to use
- Proteolytic enzymes; thiomucase
- Vitamin products: vit B1,B12
- Vaccines : ribomunyl
- Procaine and xylocaine are used in mesotherapy as vector agents
